The 25-year-old trailed closest rival Jenjira Srisaard of Thailand for most of the race before finally surging ahead in the final metres to touch the wall in a time of 26.83sec. Srisaad was a whisker behind in 26.94.
This was Quah's third gold medal at the Kuala Lumpur Games, after winning the 100m freestyle earlier in the night and the 4x100m freestyle relay on Monday.
Jasmine Alkhadhi of the Philippines finished third in 27.27sec.
Quah has one more race to go on Wednesday night — the 4x200m freestyle relay.
