Individual accountability needed to prevent recurrence

I refer to the report “AGO report flags several ministries for lapses” (July 27). Individuals should be held accountable for allowing the misuse of significant amounts of public funds.

I refer to the report “AGO report flags several ministries for lapses” (July 27). Individuals should be held accountable for allowing the misuse of significant amounts of public funds.

The response from the ministries and statutory boards involved can be summarised as “committed to rectify the lapses and putting in place measures to prevent future occurrence”.

These are aimed at work processes. But lessons will not be learnt and future occurrence will not be prevented if there is no individual accountability.

There must be greater transparency on investigations and actions taken, particularly when it comes to the relevant persons in charge, in order to be accountable the public.
